Questionnaires, the Choice activity and Polls are all used to find out student opinion.  Choice and Poll only have one question, and the student can only choose one option.  The questionnaire allows for multiple questions and question types.

POLL: is positioned in either the left or right column of a course page.  You can only have one Poll running at any one time. It is a great way of gauging student opinion.  Instructions how to set one up are below.

CHOICE: is more sophisticated, you can use it as a polling tool or you can use it to get students to sign-up for an event etc.  You can set a limit on the number of people who can choose each option offered.

QUESTIONNAIRE: had far more options than a Poll or Choice activity.  There are a whole range of different question types that can be used, radio button (multiple choice), check-box, text box, drop-down etc.  A questionnaire could be used for a "Learner Voice" type questionnaire, or to get student opinion on course based issues.
