DOCUMENT NAME AND TYPE: The document name should not contain punctuation such as ?,& etc.  Also, make sure that if you edit the document name you do not remove the document file extension.  All uploaded files should have a file extension such as .docx, .doc, .ppt etc.  If you have a Pages document you will need to convert it to an acceptable format by choosing to Export it.
